Given they had evidence that foam hit the wing on Columbia; all they had to do was have the crew get out and do a space-walk inspection to look for damage. DUH.

Challenger was avoidable but a learning experience. Columbia REALLY should have been avoided given all clues were there; sending up Atlantis to save the crew and fix the problem was more than customary to do.

Anybody watching this who happens to work in any management role should pay attention. I've watched piss poor management wreck too many good things. It's usually down to egotistical baggage. Managers are often selected on the basis that they can "manage" and all too often that means BULLY.
They don't listen. They are only interested in the sound of their own voice and they go out of their way to undermine subordinates who look like they may have a clue. It all gets worse in times of stress, like budget cuts or potentially awkward attention. That is when failure to build a team and care for the other people in it make for disaster. -
This video really illustrates the problems without our government (and large organizations, as a whole) when it comes to decision-making, transparency, communication, and accountability. And we are still making the same mistakes today...the most recent example is the F-35 fighter. A relatively recent example is the disaster response to Hurricane Katrina. The problem is that, to date, there's not been any successful solution. Today, whistle-blowers receive harsher treatment than ever before, and that mindset can be spell death for the guys on the ground (disaster response, military, LE/fire/EMS, etc.)
Without changes to how we do our strategic and operational planning, it is going to be very hard, if not impossible, to solve some of the impending problems America is moving towards (for example, the need to replace most of our water, power, roadway, bridge & dam, and communication infrastructure, as a huge portion has reached the end of safe service life.) The scariest part is how neither of our two major political parties in the US even acknowledge this issue, and instead spend their time talking about how bad the other side is, pointing the questionable alcohol-powered actions a candidate did when they were young, and focusing on stupid issues that are far less pressing. -
