The only surviving minutes on a piece of tape found on the ground ( +BBC in/outro) .. . . .►more info ► Among the 80.000 recovered items later found on the ground was this videotape recording made by the astronauts during the start of re-entry. The 13-minute recording shows the flight crew astronauts conducting routine re-entry procedures and joking with each other. None gives any indication of a problem. In this video, the flight-deck crew puts on their gloves and passes the video camera around to record plasma and flames visible outside the windows of the orbiter (a normal occurrence). This recording, which on normal flights would have continued through landing, ends about four minutes before the shuttle began to disintegrate and 11 minutes before Mission Control lost the signal from the orbiter Space Shuttle Crew: flightdeck: Commander Rick D. This disaster happened because of Nasa deciding to go politically correct and change the formulation of the foam spray-on insulation to a less structurally robust insulation for the ultimate purpose of reducing the emission of greenhouse gases, as the original foam insulation required a CFC refrigerant solvent for application. The newer, inferior insulation used a more "environmentally friendly" solvent. So ultimately the blame for this tragedy actually lies with the EPA.

NASA new about the foam hitting the wing but never told the crew. To be fair what would be the point although I assume they could have stayed on ISS and let the broken space ship float out into space or load it with explosive's on timer if that could have been a possible option but I doubt it or they would all be alive today.
