Space Shuttle Challenger blasts off from Cape Canaveral to start the STS-51-L mission. Freezing weather leads to the O-rings in one of the field joints in a solid rocket booster to leak hot gases that impinge on its mount on the external tank. However, pieces of aluminum from the solid rocket fuel build up and seals the joint. 58 seconds later, a strong jetstream dislodges the aluminum, allowing rocket fuel to leak out once more. 15 seconds afterward, the external tank breaks up, the solid rocket boosters fly free, and aerodynamic forces rip Challenger into pieces that fall into the ocean. All seven astronauts on board are killed.

The thing that is so sad is that after the shuttle exploded the crew capsule remained intact and the crew survived the explosion but were killed I think it was about 30 seconds later when they hit the ocean.

The same bullshit that happened with Challenger at NASA happened with Columbia. NASA should have made a change after Challenger and maybe Columbia wouldn't have been lost

The sad part here like they briefly touched on was they didn't learn from their mistakes. 17 years later they let their guard down and took for granted that this was a once in a lifetime mistake and 7 more people died on Columbia. Space travel is a risky business NASA knows this and they choose to ignore the warning signs. Gods Speed to the 14 people who gave their lives to the Shuttle program. Lets hope this doesn't happen to the new Space Craft that they are getting ready for the travel to Mars.

A massive headwind (equal to Katrina) as described would have almost certainly been reported by the astronauts as a violent shake and noted by the tower. It's just as likely that the exposed section of O-ring would have melted and expanded to seal the seam enough to temporarily hold everything together only to finally thin out enough to seep fuel, then give way completely when the Challenger throttled up.

Wow, after mismanagement and seven lives lost, the top NASA honcho was promoted? And the lone voice that said an impending catastrophe was at hand was brushed aside? Great recipe for a disaster, ego and gazillions of profit.
