I remember that day and seeing it happen on TV. This is about the very worst "bad day at work" you can possibly imagine. My God rest their souls.

Three words no flight director ever wants to say: lock the doors. These guys are the best of the best, and handle this tragedy with such professionalism.

NASA rehearses and prepares for everything. Which means from the time they first knew about the foam strike on launch they knew exactly what failures to expect on re-entry and had reviewed them all in preparation. Which means they watched as one by one every expected failure began to happen. And there was absolutely nothing they could do about it except pray it held together long enough...

Mission control knew about the wing damage. They felt it was an acceptable risk to try the landing instead of repairing the damage. They were wrong.
