Be sure your hands are sanitized if you don't have gloves and make sure you use antibacterial soap.And its good to use alcohol or antibacterial soap to clean the injection end

Have you thought about getting a button style tube instead of a PEG style? Or what about cloth g-tube pads? My son used to have A LOT of irritation, redness, and granulation tissue until we started using cloth g-tube pads. Everytime we see the surgeon all he talks about is how beautiful his stoma looks. lol. Your site just looks a little irritated, that's why I bring that up. And button style G-tube's seem to help with that as well (my son has the AMT Mini One, he used to have the Mini Classic)
