5 Deadliest Space Travel Incidents. Travelling to space is of course always risky and can be very deadly given the energy needed to get humans to space. Today we are listing the 5 deadliest space travel accidents. 5. The Soyuz 1 In 1967 The Soviets launched a manned spacecraft into orbit and it resulted in the first space fatality ever. Vladimir Komarov died, when during the descent, his parachute failed. As it turns out, engineers noticed hundreds of design flaws, but Soviet leaders forced the mission due to political pressure. 4. X-15 Flight 91 In 1967, during the heart of the Soviet-American space race, the Americans suffered an early fatality of their own. Michael J. Adams was one of the most experienced pilots around, but he sadly died when X-15 Flight 91 exploded during takeoff. An investigation into the incident found that Adams likely lost control of the craft, possibly due to vertigo. 3. The Soyuz 11 In 1971 the Soviets sent Soyuz 11 to the space station, and the crew worked at the station for about three weeks. The trip home proved disastrous when the crew’s capsule completely lost pressure, which ultimately killed the crew. The Craft actually returned safely to earth, but the lack of pressure in the cabin caused all three members of the crew to suffocate to death during the trip home. 2. The Challenger The Challenger broke apart on June 28, 1986 during a NASA mission, killing all seven crewmembers. Just 73 seconds into the flight, The Challenger broke apart due to a technical problem and plummeted down towards the Atlantic Ocean. The launch and subsequent explosion were captured on live television as millions of viewers had tuned in. 1. The Columbia The Columbia was reentering Earth’s atmosphere on February 1, 2003 when it fell apart. Investigators later found that he wing had been severely damaged by a piece of foam insulation during take off. The craft soon exploded, tragically killing all seven crewmembers.
